• Naval Personnel: Form 1, Form 4, DOB Certificate
• Defence Civilian: Form 1, Form 4, DOB Certificate, Proof of ID Card (ID Card to be shown at the time of admission)
• Ex-Service Men: Form 2, Form 4, DOB Certificate, and Discharge Sheet copy with the name of the child mentioned in it.
• For all categories: Form 4 submission is mandatory
